HOW DO I....
MAKE AN APPOINTMENT?

The doctors will see you in surgery by appointment which may be made by calling in at the surgery or by telephoning 023 9258 3344.

You may also ask for a telephone consultation and the GP will call you back the same day (see details below)

Normally you should book to see your own doctor, thus ensuring continuity of care. However, in an emergency or in your doctor's absence, one of the other partners will be happy to see you. If you do want to see another doctor for a special reason (for example if you want to see a female doctor) please explain this to the receptionist.

Please make follow-up appointments in advance (if requested) as spaces are limited.

If you cannot keep your appointment PLEASE tell the receptionist as soon as possible so that it may be offered to someone else and prevent long delays waiting for appointments. We run a daily duty clinic between 8.00 - 10.30am and 3.00 - 5.30pm where you will be seen by the duty team, consisting or a doctor, practice nurse and healthcare assistant, for:

a) Patients who are unwell and wish to be seen the same day.
b) Patients who are unable to obtain a suitable pre-bookable appointment with a GP within two working days.

A number of pre-bookable GP appointments are available during the sessions which may be booked 12 hours in advance (please ask if you wish to know the name of the duty doctor). Alternatively, you may attend the clinic and be given an appointment slot on your arrival.

ARRANGE A TELEPHONE CONSULTATION?

If you wish to speak to the doctor or nurse please book a telephone consultation and the doctor or nurse will call you back the same day.

HOW DO I....
OBTAIN A HOME VISIT?

If you are not well enough to come to the surgery you can ask for a home visit. Please telephone 023 9258 3344 between 8.00 and 10.00am if possible. You will be asked for a brief description of the problem so the doctor can judge its urgency and arrange home visits accordingly.

• If an urgent visit is required please tell the receptionist or nurse

• Late calls (ie after 12 noon) will be dealt with by the duty doctor

• Please remember the doctor can see several patients in surgery in the time it takes to visit one patient at home

OBTAIN TEST RESULTS?

For results of investigations please ring 023 9258 3344 after 8.30am Monday to Friday. Please allow seven days for results to come through unless advertised differently. We will only give results to the patient concerned unless given permission to do otherwise.

HOW DO I...
REGISTER AS A PATIENT?

To register with the practice you must live within the PO12 address area and provide proof of residence such as a recent utility bill or details of house purchase.

Please ask for an application form at reception, one for each member of your family. Please complete the form(s) fully, ensuring that you supply us with information about your last GP so that we may request your medical records to be forwarded to us. Any foreign national newly arrived in the country should be in possession of UK Border Agency documentation or an ID card detailing their immigration status. Failure to produce this may result in being charged for medical services. If you require assistance please ask the receptionist. If you are on repeat medication, please bring either your re-order slip or your medication with you when requesting medication for the first time.

You will be asked to complete a medical questionnaire for yourself and each member of your family and those over five years old to attend a new patient health check with one of our clinical team. Our health visitors will be informed of children less than five years of age being registered with the practice.
